Neru Phoenix stance lock

Has anyone had Neru lock in phoenix stance? We have saved and done everything imaginable to get him free but he won't come out even after about 10 more hours of gameplay. Nothing specific was going on when he got stuck and I can't figure out how to free him.

have you tried letting him die and then reviving him?
